Send a director questionnaire

Ask a director to confirm their details and declare their interests themselves, through a secure link.

Screenshots are retaken from IQ Entities on every release. Updated 14 October 2026.

When a director is appointed, and again each year, they are asked to confirm their personal details and declare their conflicts, related parties and dealings with the group. Rather than typing it in for them, send the questionnaire from IQ Entities. The director answers through a link in an email, without needing an account, and what they declare is recorded on their page.

The director needs an email address on their profile before a request can be sent.

Send the request

  1. Open the director's page and choose Information requests in the sidebar.

  2. Click Request.

  3. Under On behalf of, choose the company that is asking. The companies where the person holds an appointment are listed; if they are not appointed yet, choose another entity with the link under the list.

  4. Under Sections, keep the sections you need. Sections that do not apply are dropped when the request is sent, such as Listing Rules disclosures for a private company.

  5. Click Send request.

Track what comes back

  1. The request is listed with its status: Sent, then Opened and In progress as the director works through it, and Submitted once they have finished.

  2. Click View declaration on a submitted request to read what the director declared, and on which date.

Tips from practice

  • The director can save and finish later. A request that has been opened but not submitted shows how many answers they have given.
  • Re-send sends a new link and stops the old one working, which is the right answer when a director says they cannot find the email.
  • Use Cancel for a request sent in error or to the wrong company.
  • What the director declares is recorded straight onto their page: their related parties and their transactions with the group appear without anyone re-typing them.

Before this: Record related parties and transactions. Related: Appoint a director.
